HOPE helps fill some of the well-documented gaps in our nation’s healthcare system, with patient centered care in nursing homes via nurse practitioners. HOPE provides the missing structure to bridge multiple parties in the delivery of greater outcomes for both long-term care and temporary rehabilitation.
Dr. James L. Gardner III of New Wilmington, PA, provided the idea to create HOPE Extended Care as an outgrowth of his family medical practice. Over a period of several years, Dr. Gardner had regularly treated many of his patients in nursing homes throughout Lawrence and Mercer Counties. In time, he began to integrate the Nurse Practitioners of his practice in a mission to elevate the level of care for a particularly vulnerable segment of the population. Seeing both the need that existed and the opportunity to expand service as a separate entity, a team of investors formally incorporated the business in October 2013.
HOPE Extended Care brings daily medical treatment to patients of nursing homes. Our Nurse Practitioners collaborate in chronic disease management and needed health interventions. Functionally, HOPE is an extension to care made available by the facility and other physicians. We’re qualified to diagnose and treat illnesses in many ways including the following: performing examinations, prescribing medications, and ordering lab tests.
Current & Future Direction
In June 2014, C. Karl Sauereisen joined HOPE as President. Mr. Sauereisen’s role is to provide daily management of the company while pursuing aggressive recruitment and business development goals. A hiring campaign and target marketing program has been introduced to the Greater Pittsburgh Area. HOPE’s growth as an employer and provider of healthcare service has delivered much-welcomed Care, Collaboration and Outcomes in Western Pennsylvania. Building on success, HOPE will take its collaborative model to other states.
