Continuity of Care Proven to Reduce Hospital Readmissions

Marymount Hospital near Cleveland, OH is taking proactive measures to reduce unnecessary hospital readmissions. In 2013 they introduced Nurse Care Advocates to serve as liaisons for discharged patients who had experienced heart failure. The Advocates’ roles were mainly administrative in nature as they made follow-up calls to ensure that care, nutrition, and medication directives were being followed. As a result, readmission rates of the heart failure patients declined 26%.
